The Hub is a community-centered model designed to bring together public and allied health students, community organizations, healthcare partners, and residents to expand access to preventive services and improve coordination across systems.

Interdisciplinary student learning 

Partnering with Lehman College to train students across nursing, social work, nutrition and exercise science, and speech-language pathology in real-world, community-based practice.

Preventive health screening and education 

This initiative is built on three interconnected pillars of community-based care: health screenings, health education, and mental and behavioral health support.

SNAP Volunteer Adult Readiness Program

This program will provide those who are required to complete the volunteer or work-requirements needed for certain Medicaid beneficiaries. Individuals gain anan opportunity to build skills like teamwork and problem-solving to utilize in a public health community base setting.

Medicaid Assistance

Program

Provide free, impartial, application and renewal assistance through trained and certified personnel. Help community members understand coverage options, create or update accounts, complete applications, respond to documentation requests, all things related to renewal or recertification process.

Medically Tailored Meals (MTM) 

Initiative focuses on providing home delivered medically tailored meals to individuals diagnosed with cancer, diabetes, heart failure, and/or HIV/AIDS and who have had one or more hospitalizations within a year.
